Cardiac amyloidosis is diagnosed through both invasive and non-invasive criteria, with treatment focused on managing complications and disease-specific therapies targeting the underlying amyloid deposition.[1][2]
Diagnostic Criteria
Invasive diagnostic criteria apply to all forms of cardiac amyloidosis and require demonstration of amyloid deposits after Congo red staining in endomyocardial biopsy, regardless of left ventricular wall thickness.[1]
Diagnosis is also confirmed if amyloid deposits are found in extracardiac biopsy accompanied by characteristic echocardiographic features (unexplained LV thickness ≥12 mm plus specific findings) or characteristic cardiac magnetic resonance features.[1]

The combination of heart failure with preserved ejection fraction, increased LV wall thickness with low-voltage QRS complexes, nephrotic-range proteinuria, peripheral neuropathy, and prior bilateral carpal tunnel syndrome creates a classic pattern for cardiac amyloidosis.[1][2][8] The history of lumbar spinal stenosis further supports ATTR amyloidosis, as spinal stenosis is uniquely associated with this subtype.[2][8] The patient's age, male predominance, and multisystem involvement are consistent with wild-type transthyretin amyloidosis (ATTRwt), which typically affects men over 60 years and has a median survival of approximately 5 years from diagnosis.[4][8]
